The DD Waiver is a Medicaid funded program that provides personalized community-based services and supports to enhance the health, safety, and welfare of individuals with developmental disabilities.
What we do? We help DDW clients throughout their lives linking them to community services so they can live as independently as possible.
This position provides support coordination to individuals approved under the Developmental Disabilities (DD) Waiver program by working closely with the consumer, family, and community supports to ensure that assigned consumers are served in the least restrictive level of care.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. This part time position will be no more than 10 hours a week.
Provides case management services for individuals approved under the Developmental Disabilities Waiver.
Performs in-person assessments and planning services with individuals to determine the need for any medical, behavioral health, educational, social or other services/supports.
Performs person-centered service planning that includes development of a shared individual support plan (ISP) in accordance with the requirements of the Home and Community Based Settings (HCBS) settings regulations.
Links individuals to medical, social, educational providers, or other supports and services, including completing referrals and related activities that are required to address identified needs and preferences and achieve outcomes specified in individuals' ISP.
Assess ongoing progress and ensures that services are being delivered as outlined in the ISP, as well as addresses any change of status.
Coordinates services and service planning with other agencies and providers on individuals' care team.
Respects client confidentiality.
Maintains client charts by performing routine filing and upkeep of client binders. Keeps chart documentation in compliance and up to date according to program standards, DBHDS, CSB/BHA, and DMAS requirements. Performs chart audits as requested.
Prepares appropriate documentation for case files, billings, logs, and personal records, complying with regulations as they apply to the Developmental Disabilities Waiver. This includes forms and documentation required to engage and maintain approved services for outside agencies.
Records documentation in electronic health record systems, including the WAMS system.
Serve as point of contact for external agencies, providers and others on behalf of assigned clients.
Participates in agency committees and collects program data for agency and external partners' reports.
Performs routine day/evening travel to conduct client visits and other job duties.
Represents the Agency in the community and workplace in a professional and ethical manner.
Demonstrates sensitivity to the service population's cultural and socioeconomic characteristics.
Demonstrates strong interpersonal, decision-making, problem-solving, oral, and written skills.
Utilizes a basic knowledge of Microsoft Office applications.
EDUCATION and/or EXPERIENCE: Bachelor's Degree in a human services field is required. Must possess developmental disability work experience or have relevant education to meet knowledge, skills, and ability requirements. Must be available between 8:30am-5:00pm.
