Job Description
RN Case ManagerPosition Summary: The RN Case Manager provides case management services to the sisters living within the care residence in Merion, PA. This position works collaboratively with the individual sisters and is part of the Health and Wellness team that assesses, plans and facilitates services to meet the needs of the sisters.
Position Responsibilities:
•Case manages chronic health conditions, new diagnoses and progressive disease; develops and implements care and service plans and participates in discharge planning including attendance at care conferences, when necessary.
•Accompany/transports sisters to medical appointments/ER transfers; relay pertinent health information between Health Office and medical providers; assists/makes medical appointments.
•Conducts/participates in health assessments as required; documents health and wellness information as required.
•Provides patient education and counseling on health and wellness matters with collaboration with care team.
•Interacts with internal and external providers to ensure coordinated approach to care.
•Provides hands-on nursing care: vital signs, assessments, wound care, injections, first aid.
•Makes recommendations for referrals to specialty services such as rehab, behavioral health.
•Serve as lead for development and implementation of wellness initiatives such as programs for nutrition and fitness.
•Assist a sister with navigation through health care system/ network.
•Perceive potential health risks, problem situations and reports to supervisor.
•Demonstrates thorough knowledge of safety precautions/infection control.
Position Qualifications:
Required:
•Licensed RN
•Three to five years of strong experience in geriatric settings such as long term care, home care and experience in case management
•Comfort in working in healthcare setting serving religious women
•Ability to work independently and function in a team environment
•Commitment to holistic model of health and wellness
•Experience with management of healthcare emergencies and ability to work through crisis situations
•Strong problem-solving, decision-making, organizational and managerial skills; need for flexibility in thinking and action;
